The personal information on this form is collected under the authority of the Education Act and will only be used for
the purpose of the application to challenge for credit for a course for mature students. Should you wish to review
this information, please contact the principal of the school.
I wish to have my education and/or training credentials and related documentation assessed through the PLAR
equivalency process. I believe that my credentials and related documentation provide evidence of my prior learning
and demonstrate that I have met the curriculum expectations for each of the following courses in the Ontario
curriculum:
