Supervisor to enter reason for leave - Federation Leave - Local or Provincial Central HOLY Religious Holy Days Up to 3 days in any one work year for the observance of recognized religious holy days Paid days not granted if day not observed on the Holy day or if observed on a weekend. Enter name of holy day Central ILIF Illness – Family Up to 3 days per work year in the event of serious illness of a spouse, child, parent or sibling to arrange for alternate care. This leave is intended to provide up to 3 days leave per year for an employee to arrange for alternate care for a qualifying family member at the onset of a serious illness (including surgery, diagnosis or treatment). Enter your relationship to the family member (e.g. spouse, child, parent) Central J Special Leave Up to 3 days in any work year for matters of urgent and/or essential business including 1 day for the moving of personal effects. Upon request by Supt or Designate acceptable evidence verifying the need for the absence will be provided by the Employee. Designate = Supervisor, P/VP, or Manager. Urgent or Essential personal business includes leaves for medical, personal, or legal reasons. Annual medical/dental appts. or routine appts. related to pregnancy can be drawn from special lv. Leaves of 10+ days not entered on Form 138, Contact HR Officer Not required Central TIL Time in Lieu Employees may substitute time off in lieu of overtime worked when mutually agreed (scheduled within 30 days of the time when the overtime was earned) Must be pre-authorized by Supervisor. Enter date(s) & time(s) overtime worked (use assignment details field if needed) School V Vacation 10 & 10.5 month employees must reserve vacation days to be taken at Christmas/March break and those with insufficient days will be deducted lwop. Any vacation balances paid out in June. Vacation days must be approved in advance. 12 month employees must request permission to carry over vacation to following year . Not required Central W Inclement Weather Only with the approval of the Principal and written notification to respective Superintendent. BUDGET IMPACT SBPD SBD School Based Professional Development School Based Field Trip, Class/Office Support Approved by Principal. Replacement costs are deducted from the school budget.
