The record you are requesting will be provided electronically to the organization or institution requiring it. You will be copied on the email. This form is also available electronically here. Be advised that you must provide a copy of your government-issued photo identification (i.e. driver's license, health card, passport, etc.) to authenticate your identity. Upon issuance of the requested record, the identification document will be expunged. A.
